How do I reset my password?
Go to the Member Portal login page and Click Forgot Password. Enter the email address you signed up for at enrollment, Then Submit.

How do I make a change to my Program, like adding a dependent, going up to the next Program Tier, changing my Active Date, or changing my contact information?
A Membership Update Request Form must be signed and submitted by the Primary Member for any Program Change Request. This form is available in the Member Resources Hub.

How do I update my contact information?
To update your contact information call Member Support at (855) 699-1274.

How do I view my Member ID Card/ID Number and Membership Guidelines?
These can be accessed within your Member Portal.

How do I access my Dental, Vision, and Entertainment Discounts?
Members can register at https://wellcardsavings.com/newusers.aspx or https://www.wellcardsavings.com, by clicking “Register Now” in the top right-hand corner using their WellCard Group ID:
- AK, MA, MT, and WA Members Use Group ID: OSH to access WellCard Savings
- All other states use Group ID: ONESHARE to access WellCard Savings
- All Members will enter their OneShare Health Member ID found on their ID Card when prompted for WellCard Member ID.

How do I access Virtual Urgent Care (Telemedicine) through Clever Health?
Members can download the Clever Health App by scanning the QR code using the camera on their mobile phone.

Can I submit a sharing request in the Member Portal?
You may fill out and submit an Expense Sharing Form (Available in the Member Resources Hub), and fax or mail your completed form (CMS HCFA 1500 and/or UB-04 form, this standardized form can be obtained from your provider) to OneShare Health.

Where can I update my method of payment?
To update your payment method please call Member Support at (855) 699-1274, Monday-Friday, 8AM – 5PM CST.

Where can I search for a Provider?
Login to the Member Portal and select My Choice Provider Locator or Find a Provider.
Our Members have access to an easy-to-use Provider locator tool from First Health to find network Providers. Members are not required to use a network Provider, but you could end up with additional sharing responsibility if you choose an out-of-network Provider.
To search for a Provider, go to mychoice.onesharehealth.com. If you need any help finding a Provider, contact OneShare Health Member Support at (855) 699-1274.
